Young people with disabilities emerging from incarceration face additional barriers to a successful transition to adulthood. Post secondary education and employment are likely to be positive factors against recidivism. This article describes an example of a project about transition from incarceration to the community, targeting young people with a mental disorder or with special education needs. It looks at career development strategies, which include the role of a transition specialist in job development and the service delivery model, youth-focused planning and job matching and job recruitment strategies.
